Health Care Power of Attorney (write-in-the-blank forms)

A Health Care Power of Attorney is a legal form that lets you appoint a health care agent to help you and make health care decisions for you. This packet includes 2 optional forms to give additional instructions about specific health care treatments and related decisions.
