Petition for Name and Gender Change for an Adult (write-in-the-blank)

Before You Start

** Disclaimer (Updated on 5/26/22) **

The law that tells you how to change the sex marker on your birth certificate is currently being challenged in court.  Montana DPHHS has adopted a temporary emergency rule that does not allow changes to the sex marker on your birth certificate. Until the lawsuit is over, you can't use these forms to change the sex marker on your birth certificate.

Summary

You can use the Petition for Name and Gender Change for Adults to ask a judge to legally change your name and gender. You must file the forms with the Clerk of District Court in the County where you reside. The forms come with instructions to help you fill them out correctly.
